Jared L. Ostroff received his Pharm.D. degree from Albany College of Pharmacy & Health Sciences in 2010. Prior to completing his residency training, Jared worked as a pharmacy manager for CVS/Caremark in New Jersey. Dr. Ostroff completed a PGY1 Pharmacy Practice Residency at the Jesse Brown VA Medical Center in Chicago, Illinois, and a PGY2 residency in Geriatrics at the Veteran’s Affairs Hospital in Durham, NC. Dr. Ostroff also served as an adjunct faculty member at the University of North Carolina Eshelman College of Pharmacy during his PGY2 residency. He became board certified in ambulatory care (BCACP) in 2015. Dr. Ostroff’s areas of interest are geriatrics, transitions of care, and veterinary pharmacy.

Practice Area

Dr. Ostroff’s clinical practice site is ambulatory care at Holyoke Health Center in Holyoke, MA, where he sees patients in MTM clinic for chronic disease state management. He also works alongside the providers in clinic seeing patients after hospital discharge. He works closely with primary care providers and patients to ensure optimal medication adherence and evidenced based medicine.
