Community Patient Care Center (CPCC)

Established in 2014, the primary focus of the Community Patient Care Center is to provide individualized clinical pharmacy services to Western New England University employees and greater Springfield area patients by physician referral. Enhancing patient care and providing preventive services are the cornerstones of the emerging roles of pharmacists. These services include both weight management and Medication Therapy Management (MTM). The clinic director is Melissa Mattison, BS Pharm, PharmD, clinical assistant professor of pharmacy practice.

Weight Management

Weight management focusing on lifestyle modifications and wellness is a dynamic program offered at the clinic. Through weekly visits, food logs, increased physical activity, nutritional counseling, and the employment of strategies to overcome food challenges, the faculty motivates patients to adopt lifelong healthy habits resulting in weight loss and improved health.

Medication Therapy Management

Medication Therapy Management is a comprehensive program in which the faculty pharmacist takes responsibility and accountability for all medication-focused patient-care services. MTM is an all-inclusive review of the entire patient’s medication regimen (medication reconciliation and evaluation) to optimize the patient’s response to medications while avoiding interactions or complications. The faculty member will work closely with your healthcare provider to provide the best patient care possible.
